      <title>⁯</title><p><bold><italic>Dear Editor,</italic></bold></p><p>Recently, Emamat and colleagues published a review about NAFLD and a possible role of artificial sweeteners as a risk factor (Emamat et al., 2020[<xref ref-type="bibr" rid="R3">3</xref>]). NAFLD is the most frequent liver disorder in industrialized countries, which affects &#x7E;25 &#x25; of the population (Younossi et al., 2018[<xref ref-type="bibr" rid="R24">24</xref>]; Friedman et al., 2018[<xref ref-type="bibr" rid="R4">4</xref>]). In the past years, the prevalence of NAFLD increased in adults and in children and is also present in &#x7E;7 &#x25; of lean persons (Romero-G&#xF3;mez et al., 2017[<xref ref-type="bibr" rid="R17">17</xref>]; Younossi et al., 2018[<xref ref-type="bibr" rid="R24">24</xref>]). Moreover, NAFLD represents a risk factor of primary liver cancer (AISF, 2017[<xref ref-type="bibr" rid="R1">1</xref>]; Tr&#xE9;po and Valenti, 2020[<xref ref-type="bibr" rid="R21">21</xref>]). In their review, Emamat et al. discuss the hypothesis that artificial sweeteners increase the risk of NAFLD (Emamat et al., 2020[<xref ref-type="bibr" rid="R3">3</xref>]). Artificial sweeteners or sugar substitutes are increasingly consumed to reduce caloric intake (Kakleas et al., 2020[<xref ref-type="bibr" rid="R14">14</xref>]; Suez et al., 2015[<xref ref-type="bibr" rid="R20">20</xref>]; Ruiz-Ojeda et al., 2019[<xref ref-type="bibr" rid="R18">18</xref>]; Uebanso et al., 2017[<xref ref-type="bibr" rid="R22">22</xref>]). The authors discuss the currently available evidence that artificial sweeteners alter the gut microbiota, which may increase the prevalence of NAFLD. </p><p>Currently, much experimental effort is invested to gain a deeper understanding of liver disease (Jansen et al., 2017[<xref ref-type="bibr" rid="R13">13</xref>]; Godoy et al., 2013[<xref ref-type="bibr" rid="R8">8</xref>], 2015[<xref ref-type="bibr" rid="R9">9</xref>], 2016[<xref ref-type="bibr" rid="R10">10</xref>]; Ghallab et al., 2016[<xref ref-type="bibr" rid="R5">5</xref>], 2019[<xref ref-type="bibr" rid="R7">7</xref>][<xref ref-type="bibr" rid="R6">6</xref>]; Vartak et al., 2016[<xref ref-type="bibr" rid="R23">23</xref>]) and to identify compounds that cause an increased risk of hepatotoxicity (Grinberg et al., 2014[<xref ref-type="bibr" rid="R12">12</xref>], 2018[<xref ref-type="bibr" rid="R11">11</xref>]; Albrecht et al., 2019[<xref ref-type="bibr" rid="R2">2</xref>]; Kim et al., 2015[<xref ref-type="bibr" rid="R15">15</xref>]). Research in this field is often hampered by difficulties to extrapolate data from animal or <italic>in vitro</italic> experiments to the <italic>in vivo</italic> situation (Schenk et al., 2017[<xref ref-type="bibr" rid="R19">19</xref>]; Leist et al., 2017[<xref ref-type="bibr" rid="R16">16</xref>]). The review of Emamat et al. clearly shows that there is strong evidence that artificial sweeteners influence the composition of gut microbiota. However, further work including prospective and intervention studies are required to clarify if this mechanism really causes an increased risk of liver disease.</p></sec>
